Reiko, I hadn't heard about it, so thank you for providing that link.

I'm curious to know what happened. It sounds like equipment malfunction isn't entirely likely. My paranoid fiction-writing brain immediately tosses hijackers into the bridge to distract the captain and crew...

It certainly is peculiar. After all, it doesn't sound much like human error, and equipment malfunction has been deemed unlikely. I'm amazed that they've accounted for nearly everybody.
