I've never known this, and just muddled through: How much underlining of nearby punctuation do you do?

Do these need correcting?

"Will you quit _now_?" [or does the ? get underlined too?]

_"It's about to explode!"_

"I served on the _Saratoga_."

i never underline punctuation... it just looks weird to me.

i'm probably not doing it right, but i don't particularly care.

The only one of your suggestions I would change is the second, which I would punctuate thus:

"_It's about to explode!_"

I include end punctuation if the entire sentence is italicised, but the quotation marks seem to me to be in a different context, so I would only italicise them if they were included themselves in text that was italicised:

_He said "tomorrow"._
