I find focusing on something that takes some effort but is still within the resources of my memory will get my mind of less pleasant things. Like running through the scene sequence of a movie, or hymn lyrics that I only almost know.
I keep feeling like it would be better to meditate on nothing, but I don't really know how to do that without whatever mood I was trying to overcome crashing back in on me.
I like walking as a way of meditating.

I used it to give birth naturally. I don't know what you mean by "does it work". It didn't do miracles, it didn't give me a painless childbirth, but it helped make it bearable.
Edit: Oh, let me answer your last question. I used music and relaxation. The music was very pleasing to me and very familiar, and I did a lot of practice. Porter helped me practice by giving me an "indian burn" while in a trance. I remember being disappointed that I could feel the indian burn. But I was no longer disappointed when he did it to me again out of the trance. DANG!! That hurt! While in a trance, it was just a sensation that I could deal with. Out of the trance, it was ouchy ouchy.
