So, the magazine I wish to submit to calls for a cover letter which contains information about myself and any publication credentials I have.

I don't want to cover page to look hokey. Does anyone know what the correct format for a cover page is?

No! Those are not the formats you should be using for a submissions cover letter. Here, try this instead.

Edit: Whoops! Wrong link (though there is some information on cover letters there). Let me dig about and find the correct one.

Something in that format should be adequate. If you do not have publishing credits relevant to the magazine you're submitting to, simply omit them; if you are previously unpublished, say so. There is no need to mention what rights you are offering unless your story is a reprint and you are submitting to a magazine that accepts reprints.

I'm probably missing something out, but I'm sure someone will correct me if that's the case.

I always start out with a statement of my innocence. You shouldn't beat around the bush. Then say that even if you HAD done it, there's not really a problem because it was the right thing to do under the circumstances. Close with a commitment to not rest until you find the guilty party(ies) and ensure that they are brought to justice.
