They did it again, and restricted our internet access to intranet access. I am now forced to sneak my laptop into work and use our handheld ordering device's fax line to dial up my internet provider.

I will try to be on as much as circumstances permit...

Currently, I am working 6 days a week, mostly 8 and 12 hour shifts. If I don't post at work, I usually collapse at home, or catch up on video games and movies [Big Grin]

Hey Alucard, I have drug question...

I had a young adult patient who was morbidly obese, with hypertension, sleep apnea, and insulin resistance. Also has hypothyroidism in good control on Synthroid. After various discussions I prescribed Meridia for her. Before she filled it she asked her trusted pharmacist for advice. He told her that I had no business prescribing a dangerous drug like Meridia, especially to someone on a thyroid medication. She threw out the prescription immediately, of course.

Is there something I need to know about the combination of meridia and hypothyroidism?

Because I thought this was quite annoying. If he was that concerned, he should have called me.

If you run a google search for "hypothyroidism meridia" you get a lot of lawyer's websites. Unfortunately, the only information listed is that "organic causes for obesity (eg hypothyroidism) should be ruled out before prescribing" this drug.

Yesterday I was told that the whole "antibiotics can cause oral birth control pills to be ineffective" thing was a huge myth that had snowballed without any evidence to support it. [Confused]

Maybe there is a rash of stupidness going around, Theca.

Actually, there is a lot of talk this month about antibiotics (other than rifampin) and whether they affect birth control or not. I'm not altogether clear where these stories came from, and I'm not really going to look it up right now because I think it's safer to assume that there MIGHT be an interaction in some people. I am pretty sure I even got an informational email about it from some physician website, but I erased it already.

Wow. I queried our hospital pharmacist earlier & she was as perturbed as I, but apparently this is widepread in discussion. Unfortunately, the medical literature on the topic is scant.
