You prepare a note in writing stating, "effective <<DATE>> I will terminate my employment with you. It's been a pleasure working with you." Then call your manager over, tell him your leaving, and ask him if he wants it in writing.
It's customary to provide two weeks notice, but be financially ready to be asked to leave immediately.
Be courteous - you don't want to destroy a potential reference. If there is anything that might convince you to stay, know what that is before talking to the manager. Don't bring it up, but be ready to respond if he asks.

I'm just tired of my job. Makes me feel like a terrible person because really I should suck it up and deal with it, but it is not getting better, it will not get any better. I will not get promoted on this thing. I will get stuck bagging and cleaning up toilets and this is not really what I want to be doing right now. Plus the pay is not so good and there are certain addictions that I have... But, at this moment it is impossible.. >..<

I actually just went through this. For me, I just told him I had to quit to focus on my school and my writing. He said okay. But that was work for a small business. Sounds like you work for a store of some sort. In that case you probably need to give two weeks notice and look for a different job during that time.

Currently I work at a supermarket. I have already begun filling out applications in various places, but now I have no savings at all. I cannot save money on this job. I have nothing solid pending. I honestly wish I could just quit, become focused, write more, do SOMETHING constructive! Hopefully I can get that job near my house, but the more I think of it, the less I want to work retail ever again... I ought to simply suffer in silence until January, but my wrists can't take much more of this.
