I am trying to create a document that I can repeatedly use to make log notes. I have formatted the document with the premade fields I want to be on every document. I have created blank underlined segments where I will type log information for each unique log note.

My question is, how do I format this so that when I start typing on an underlined spot, the underlined portion will remain under the words I am typing, without pushing other spaces and tabs ahead, since this would mess up the rest of the document?

For example, the template looks like this:

Brief Narrative of Client Progress Toward Objectives:_______________________________________________________________________________________


...and I want it to look like this after making some notes (except the actual note I made would also be underlined, I just don't know how to do it in Hatrack format):

Brief Narrative of Client Progress Toward Objectives: The client is doing great.__________________________________________________________________


I want to be able to do this my simply typing on the underlined portion, without having to make any other adjusments. In other words, each underlined space I type over should be replaced by an underlined letter, without affecting any other tabs or formatting in the document. I have seen this done before. The template had sections where you could enter text, and these sections were grayed out until you typed in them. None of the other formatting was affected by the length of what you typed (as long as it wasn't longer than the allowed space). I just don't know how it was done.

I hope my question makes sense and isn't too confusing. It's hard to describe without visuals.

It's called forms.

This might help. Or google "MS Word forms" and you'll find lots of helpful stuff.
