Such things as cover letters have been discussed here, but what is the correct format for a cover page with such imformation as story title, authors name, address and the like.
Usually, I centre the story title and author name, then put my address, phone number and e-mail address top right corner, and word count at the bottom. Is this correct, or should I be doing it a differnt way?

As long as you have the important information somewhere on the first page, the editor is not likely to toss your manuscript without reading it.
That said, following manuscript format as closely as you can (because that's what editors are used to looking at for such information) is the safest bet. Try not to make the editor work any harder than necessary.

The website of the Science Fiction and Fantasy Writers of America has some great information on all aspects of writing.
If you go here and scroll down to the section on Manuscript Preparation, you will find quite a few articles on the subject. I recommend Vonda McIntire's article especially.
