Any art on the cover should look better than what I could do.
If it looks anything like the cover of a romance novel, I won't buy it (exception: The guy on _A Game of Thrones_ looks kind of like Fabio, but the book's awesome) http://www.georgerrmartin.com/gallery/thrones04.html
The cover/spine should make it clear who the author is and what the title is. They should be easy to read.
No puns. (rare exceptions: Robert Asprin and Terry Pratchett)
Nice typesetting is a plus.
Nonsensical pictures that obviously have nothing to do with the story is a minus.

When I publish a novel, I pray, PRAY that I quickly get say in the cover design. Bad covers make me cry a little bit inside.
