Unless you're fooling about with writing present tense prose, it should be "had".

"Such a choice had its consequences."

I would change this to "Such choices had their consequences." You might not have noticed, but that line is in the simple past tense, which would place it in the present of your narrative. By generalizing it by use of the plural, you avoid the necessity of doing something barbaric like "Such a choice [was still having/had had] its consequences."

I'd go with everybody else and say it's "had".

Also, it's GRAMMAR and not grammer.
